Instructions
Please see the list of documents to the right. Open the "Table of Contents", then all subsequent documents. Read through all the documents, but only print the pages that contain areas where you need to fill out information and/or sign.
If you have any questions while you are filling the forms out, please feel free to contact me by phone, text, email, or we can ZOOM online at your convenience.
Submit all printed pages, documents and fees to PO Box 775, Lee, FL 32059. Click here for options to submit fees or make your check payable to "Family Tree Private School".
After your application, documents and fees are received we will complete your enrollment and you will have access to our Welcome Packet and other supportive documents and links; and we will communicate so I can answer all your questions and review the Monthly Reports you will submit (they take about 5 minutes to fill out). We will work together to make sure you are off to a good start in your homeschooling program and receive all the continued support you need as you educate your children.
